ProMedica Hospice, formerly Heartland Hospice, is currently searching for a full-time Social Worker to support hospice patients in their homes in the Rockford and surrounding areas.
The Social Worker is responsible for providing clients and families psychosocial care regarding economic, social, and emotional needs.
This is mainly a weekday position with limited on call.
This position requires a bachelors in social work and certification or licensure in the state of IL and/or WI.

Salary:
$30 per hour.
Estimated Salary: $20 to $28 per hour based on qualifications.
